Diastolic Dysfunction in Patients with HFpEF e EDPVR is shied up and to the le in these patients with HFpEF. e poor diastolic compliance of the le ventricle is also reflected by the stiffness constant derived from the same data. Reprinted with permission from Zile M et al. N Engl J Med 2004;350:1953–1959. Figure 6. Relationship Between Resistance and Compliance in the Pulmonary Circulation In the pulmonary circulation, resistance and compliance are inversely related, as illustrated from this plot of patient hemodynamics taken from multiple catheterization laboratories. e product of resistance (R) and compliance (C) is remarkably constant over age or interstitial lung disease. However, the RC relationship moves to the le with increasing PCWP, suggesting increased pulsatile relative to resistive aerload to the RV when there is pulmonary congestion. e arrow represents a theoretical heart failure patient with PH out of proportion to the PCWP.
