Each of the 22 items is rated on a scale from 0 to 5, with 0 indicating no problem and 5 indicating the most severe problem. Total scores range from 0 to 110. Additional scoring is available for productivity and allerg y. 8.9, 12 a Rhinosinusitis Disability Index (RSDI) A questionnaire measuring the impact of rhinosinusitis on physical, functional, and emotional aspects of daily life. Responses to 30 items are scored from 0 to 4, with higher scores indicating greater disability. Total scores range from 0 to 120. 10.35 CRS PRO Questionnaire defined with patient input per Food and Drug Administration guidance on patient-reported outcome measures acceptable for use as end points in clinical trials. 12-Item Likert scale survey with scores ranging from 0 to 4, from symptoms not bothersome at all to 4 as the most severe.
