Appendices 38 Appendix A: Antipsychotic Agents Antipsychotic a Generic Name (Brand Name) Indications and Dosage For all antipsychotics listed below: Black Box Warning - Increased mortality in elderly patients with dementia-related psychosis Aripiprazole (Abilify ® ) Adjunctive therapy for depression (used with an antidepressant): • Oral: 2 to 5 mg/day (max: 15 mg/day) Acute Agitation: • IM: 5.25 to 15 mg once; a repeat dose up to 5 mg ; may be given at >2 hour intervals. (max: 30 mg/day) Agitation associated with dementia (off-label): • Oral: 2 to 5 mg daily (max: 15 mg daily) • IM: 2.5 to 10 mg once; a repeat dose of up to 5 mg may be given at >2 hour intervals (max: 15 mg/day) • If no response in 4 weeks, begin taper to discontinuation. Clozapine (Clozaril ® ) Psychosis in Parkinson Disease (off label): • Oral: 6.25 mg daily in 1 or 2 divided doses, increase based on response and tolerability every 3 to 7 days (max: 50 mg per day) Treatment-resistant agitation associated with dementia (off-label): • Oral: 12.5 mg at bedtime titrate by 12.5 mg every 3 to 5 days as needed
