25 Process Quality Indicators Outcome Quality Indicators • Percentage of individuals within the facility at a specific point in time with a diabetic foot ulcer and/ or diabetic patients who are malnourished (point prevalence). • Percentage of individuals who did not have a DFU on admission who acquire a DFU during their stay in the facility (facility- acquired rate). • Ensure every patient is assessed for malnutrition and malnutrition risk • Ensure malnutrition prevention plan is documented, implemented and modified in response to changes • Ensure any DFUs are assessed and finding documented at least weekly to monitor healing • Individualized treatment plan and goals are available for each patient • Every individual at risk receives appropriate screenings and assessments, and that the plans are well documented • Patients receive appropriate education on nutrition, self-care and psychosocial support • Measurement is regularly conducted and reported to stakeholders
