Table 1. Criteria Proposed for Clinical Diagnosis of the Metabolic Syndrome
AHA/NHLBI1 Clinical Measure : Any 3 of the Following 5 Features
Waist circumference ≥ 102 cm in men or ≥ 88 cm in women (non-Asian origin); ≥ 90 cm in men or ≥ 80 cm in women (both East Asians and South Asians)
IDF2
≥ 94 cm in men or ≥ 80 cm in women (Europids, Sub-Saharan Africans, and Middle Eastern); ≥ 90 cm in men or ≥ 80 cm in women (both East Asians and South Asians; South and Central Americans); ≥ 85 cm in men or ≥ 90 cm in women (Japanese), plus any 2 of the following:
Triglycerides (fasting) ≥ 150 mg/dL or on drug therapy for high triglycerides
HDL-C Blood pressure Glucose (fasting)
< 40 mg/dL in men or < 50 mg/dL in women or on drug therapy for low HDL-C
≥ 130 mm Hg systolic or ≥ 85 mm Hg diastolic or on drug therapy for hypertension
≥ 100 mg/dL or drug therapy for elevated glucose
≥ 150 mg/dL or on drug therapy for high triglycerides
< 40 mg/dL in men or < 50 mg/dL in women or on drug therapy for low HDL-C
≥ 130 mm Hg systolic or ≥ 85 mm Hg diastolic or on drug therapy for hypertension

(1) AHA/NHLBI (American Heart Association/National Heart, Lung, and Blood Institute) criteria for the metabolic syndrome are used in this guideline to define metabolic risk. (AHA/NHLBI) These criteria are essentially the same as the harmonized criteria proposed by AHA/NHLBI and IDF in 2009, with the exception that the ethnically based waist circumference cut-offs adhere to the original AHA/NHLBI criteria. Further study is necessary to identify optimal ethnically based waist circumference criteria. (2) IDF = International Diabetes Foundation
