• Central obesity + 2 risk factors
• Ages 10–16 years:
▶ WC ≥90th percentile for age & sex; adult cut-off if lower
▶ SBP ≥130 or DBP ≥ 85 mmHg
▶ TG ≥ 150 mg/dL
▶ HDL <40 mg/dL
▶ FBS ≥100 mg/dL
• Adolescents >16 years:
▶ WC ≥94 cm for males; ≥80 cm for females
» Individuals who self identify as Asians, Central/
South American: ≥90 cm in males, ≥80 cm in females
▶ SBP ≥130 or DBP ≥85mmHg or previously diagnosed HTN
▶ TG ≥150 mg/dL or treatment for hypertriglyceridemia
▶ HDL <40 mg/dL for males; <50 mg/dl for females or
tx for low HDL
▶ FBS ≥100 mg/dL or known T2DM
• <10 years, not recommended to use
• >16 years, adult criteria recommended.
➤ Clinical Implications
• MetS in adults is linked to a two- to three-fold higher ASCVD risk,
a f ive- to ten-fold higher T2DM risk, and a greater likelihood of
MASLD, polycystic ovarian syndrome, obstructive sleep apnea, and
certain cancers.
