Process indicators Outcome indicators • Percentage of patients assessed for predisposing and precipitating risk factors for delirium • Percentage of patients assessed for potential causes of delirium • Percentage of patients for which non- pharmacologic strategies for delirium prevention were implemented • Percentage of patients who were treated with antipsychotics for delirium only • Incidence and prevalence (present on admission to facility) of delirium Dementia Monitor facility performance in management of dementia and BPSD. Process indicators Outcome indicators • Percent of patients screened for dementia • Percent of patients with dementia who are on an antipsychotic • Percent reduction and/or stopping of antipsychotics as part of GDR • Adverse outcome events in dementia care, fall with injuries, hospitalization, aspiration pneumonia Depression Monitor the facility performance in the treatment of depression. Process indicators Outcome indicators • Implementation of facility protocols consistent with current standards of practice for the recognition, assessment and treatment of depression • Percentage of patients assessed for risk factors and/or screened for depression • Percentage of patients with depression who receive interventions consistent with current standards of practices • Percentage of patients with a noticeable improvement in target symptoms after treatment of depression • Percentage of patients treated to remission (at least 3 weeks of the absence of both sad mood and reduced interest and no more than three remaining symptoms of the major depressive episode)
