signs concerning for malacia
(hypoxemia, desaturation
episodes, recurrent cough,
dyspnea, wheezing, and/or
inability to wean PPV)

diagnosed by imaging
If bronchoscopy not feasible and available,
consider unsedated free breathing Chest CT
(or CT Angio or MRI)
Consider tracheostomy/airway intervention,
PEEP titration
