Key Points Antihyperglycemic Therapy Glycemic Targets ÎÎHbA1c < 7.0% (mean plasma glucose ~150-160 mg/dL [~8.3-8.9 mmol/L]) ÎÎFasting and premeal glucose < 130 mg/dL (< 7.2 mmol/L) ÎÎPostprandial glucose < 180 mg/dL (< 10.0 mmol/L) ÎÎIndividualization is key: •  More stringent targets (HbA1c 6.0-6.5%) – patients with short disease duration, long life expectancy, no significant cardiovascular disease •  Less stringent targets (HbA1c 7.5-8.0%+) – patients with history of severe hypoglycemia, limited life expectancy, advanced complications, extensive comorbid conditions ÎÎAvoidance of hypoglycemia Figure 4.
