Warnings and Precautions Contraindications • Should not be used with other drugs for weight loss • Pulmonary hypertension (use >3 months associated with a 23-fold increase) • Valvular heart disease • Treatment should be continued only if the patient has lost at least 4 pounds in first 4 weeks • Tolerance to anorectic effect • May impair the ability to engage in potentially hazardous activities • Dependence with withdrawal syndrome on cessation • Hallucinations • Toxic psychosis • Possibility of adverse interactions with alcohol • Pulmonary hypertension • Advanced arteriosclerosis • Hyperthyroidism • Known hypersensitivity or idiosyncrasy • Glaucoma • Severe hypertension • Agitated states • History of drug abuse • Use in combination with other anorectic agents • During or within 14 days of taking MAOIs • Can decrease cyclosporine exposure • Take a multivitamin supplement • Rare cases of severe liver injury • Monitor renal function in patients at risk for renal insufficiency • Weight loss can increase the risk of cholelithiasis • GI events with a diet high in fat • Cyclosporine (taken 2 hours before or after orlistat dose) • Chronic malabsorption syndrome • Pregnancy and breastfeeding • Cholestasis • Levothyroxine • Warfarin • Antiepileptic drugs • Known hypersensitivity • See Orlistat prescription • See Orlistat prescription • Boxed Warning : ▶ Suicidal behavior and ideation • Warnings: ▶ Risk of seizure (adhere to dosing schedule and avoid coadministration with high-fat meal) ▶ Increase in blood pressure and heart rate ▶ Hepatotoxicity ▶ Angle-closure glaucoma ▶ Weight loss may cause hypoglycemia • Uncontrolled hypertension • Seizure disorders • Anorexia nervosa or bulimia • Drug or alcohol withdrawal • MAO inhibitors • Chronic opioid use • Known allerg y • Pregnancy Over-the-counter (Alli), Orlistat prescription (Xenical), Phentermine/topiramate (P/T) (Qsymia)
